Robotic Assisted Direct Anterior Hip Replacement (DAA)

Total Hip Replacement

Introduction

Total Hip Replacement surgery is a life-changing procedure for patients suffering from arthritis, avascular necrosis, hip fractures, or other degenerative hip conditions. While traditional hip replacement techniques have been successful for decades, the Direct Anterior Approach (DAA) has emerged as a less invasive, muscle-sparing alternative.

What is Direct Anterior Hip Replacement (DAA)?

The Direct Anterior Approach (DAA) is a minimally invasive technique where the surgeon accesses the hip joint from the front (anterior) of the thigh rather than the conventional way of accessing the hip via the side (lateral) or back (posterior). This approach avoids cutting major muscles but instead separates the muscles. This results in lesser postoperative pain, faster recovery, improved joint stability and improved mobility post-surgery.

Key Features of DAA:

✔ Muscle-Sparing – No muscles are cut, only gently separated.
✔ Smaller Incision – Typically 3-5 inches long.
✔ Less Postop pain- Less postoperative pain and therefore less painkillers needed.
✔ Faster Recovery – Many patients walk the same day.
✔ Lower Dislocation Risk – Preserves natural hip stability.

Advantages of Direct Anterior Hip Replacement (DAA) vs. Conventional Approaches

Feature Direct Anterior Approach (DAA) Traditional (Posterior/Lateral) Approach
Incision Location Front of the hip Side or back of the hip
Muscle Damage Minimal (no muscles cut) Moderate (some muscles detached)
Hospital Stay Often 1-2 days Typically 2-4 days
Recovery Time Weeks Weeks
Dislocation Risk Very low (~1%) Higher (~2-5%)
Restrictions post operative Less restrictions More restrictions especially in deep hip flexion
Physical Therapy Less intensive More intensive

What is Robotic Assisted Direct Anterior Hip Replacement (RA-DAA)?

It’s the performing of direct anterior approach (DAA) total hip replacement utilizing state of the art Robotic technology. The Robotic Arm enhances surgical accuracy by:
✔ Creating a 3D surgical plan tailored to your anatomy
✔ Allowing real-time adjustments during surgery
✔ Ensuring optimal implant positioning for long-lasting results

Why Robotic Assisted Direct Anterior Hip Replacement (RA-DAA)?

It combines the best of both worlds by bringing precision of performing hip replacement using Robotic technology to the minimally invasive nature of Direct Anterior Hip Approach (DAA). This allows precision, accuracy and fast track recovery after a total hip replacement. It also eliminates the need for intraoperative imaging that’s used in conventional direct anterior hip replacement (DAA).

Feature Robotic Assisted Direct Anterior Approach (RA- DAA) Conventional Direct Anterior Approach (C- DAA)
Incision Front of the hip Front of the hip
Technology Robotic Assistance Intraoperative X-rays/ Image Intensifier
Additional Pins (and 2-3 extra stab incisions) are placed in the pelvis for Robotic visualization Extra Pins (and incisions) are not needed
Acetabular Cup Placement More Precise Less Precise
Overall Leg length/ Offset restoration More accurate Less accurate
Intraoperative Xrays/Radiation Reduced or Not needed Required
Costs More Costly Less Costs involved

Are you struggling with hip arthritis pain? Walking, sitting, or even sleeping may feel like a challenge. You may need a hip replacement.
Direct Anterior Hip Replacement (DAA) with Robotic Assistance offers:
✔ Faster recovery – Many patients walk the same day
✔ Minimal muscle cutting – Preserves strength & stability
✔ Precision implant placement – Reduced risk of dislocation
✔ Less pain – Quicker return to daily activities

Frequently Asked Questions (FAQs) About Direct Anterior Hip Replacement (DAA)

  1. Who is a candidate for Direct Anterior Hip Replacement (DAA) or Robotic Assisted Direct Anterior Hip Replacement (RA-DAA)?

Any patients with hip arthritis, avascular necrosis or fractures are eligible, but those with severe deformities or prior hip surgeries may require additional evaluation.

  1. Is there anyone not suitable for Direct Anterior Hip Replacement(DAA)?

Although not an absolute contraindication, very muscular and severely overweight patients may find the approach more challenging.

  1. How long does the surgery take?

Typically 2-3 hours, similar to traditional hip replacement.

  1. Will I need physical therapy after surgery?

Yes, but less intensive than with traditional approaches. Many patients start walking the same day.

  1. What is the success rate of DAA?

Over 95% success rate, with most patients regaining full mobility.

  1. Are there risks with the anterior approach?

All surgeries carry risks, DAA has been reported to have slightly higher risk of fractures, wound infection rates, and nerve injury to the lateral femoral cutaneous nerve and the femoral nerve but DAA has lower risks of dislocation, sciatic nerve injury and muscle damage compared to traditional methods.

  1. Is Robotic Surgery better?

Yes! Robotic assistance improves precision, reducing risks like leg length discrepancy and implant misalignment.

  1. How fast do I recover after surgery?

Most patients can walk on the same day of surgery and stay 1 to 2 nights in hospital. Full recovery takes 4 to 6 weeks

  1. How soon can I drive after surgery?

Many patients can drive within 2-4 weeks, depending on recovery progress.

  1. Will my implant last as long as with traditional surgery?

Yes, implant longevity is similar (often 15-20+ years).

Patient Success Story

*”Dr. Kelvin Tan performed a Robotic Total Hip Replacement on my left hip. On July 8th, 2024, I stayed ONE night after surgery and faithfully followed through with the physiotherapy exercises for the next three weeks. Unbelievably, there was no pain, and I was off all strong painkillers after 2 weeks. On the fourth week, I could return to driving. Within 2 months, I was able to go back to swimming. After 6 months, I could challenge myself to climb Mount Faber. Thank you, Dr. Kelvin Tan, for your skill and follow-through care. This 82-year-old grandmother has never felt stronger!”*

Conclusion: Why Choose Robotic Assisted Direct Anterior Hip Replacement (RA-DAA)?

The Robotic Assisted Direct Anterior Approach (RA-DAA) hip replacement offers a precise hip replacement that’s safer, faster, and less painful compared to traditional hip replacement. With no muscles being cut, a smaller incision, and a lower dislocation risk, patients can return to their daily activities much sooner. Combined with Enhanced Recovery after Surgery (ERAS) techniques, patients who undergo Robotic assisted Direct Anterior Hip Replacement (DAA) typically recover fast and feels more natural after a hip replacement surgery.
